What is all about?

The employee evaluation template captures the performance, skills, and professional development of the employee within the company. It helps identify strengths, pinpoint areas for improvement, and enhance communication between employees and management.

 

Key paragraphs of the template

At the beginning, the employee’s basic information should be provided, such as their name, department, position, evaluation date, email, and supervisor or manager. This serves as an introductory section to identify the employee being evaluated. Only relevant details should be included; if certain fields (e.g., department, supervisor) are not applicable, they can be excluded.

Next, the evaluation proceeds with sections where the employee is assessed based on specific criteria using an evaluation scale. More specifically:

 

Section: Work Performance

In this section, the employee’s performance is evaluated in key areas of their job, their communication and collaboration with colleagues, as well as their ability to demonstrate creativity and decisiveness. The evaluation is done by rating the different aspects. The rating scale can range from 1 (equal to Below Expected Level) to 5 (equal to Excellent Performance).

1. Job Performance
  • Quality of Work: Rates the accuracy, consistency, and overall quality of the employee’s work.
  • Productivity: Evaluates the amount of work completed within a given period.
  • Dependability: Measures reliability in meeting deadlines and responsibilities.
  • Problem Solving: Examines the ability to identify and resolve work-related challenges.

 

2. Teamwork and communication
  • Collaboration: Assesses the employee’s ability to work effectively with team members.
  • Communication Skills: Evaluates clarity, professionalism, and effectiveness in communication.
  • Adaptability: Rates the employee’s ability to adjust to changes and perform under pressure.

 

3. Initiative and innovation section
  • Creativity: Measures the ability to introduce new ideas or solutions.
  • Decisiveness: Evaluates the ability to take initiative and propose improvements.

 

Section: Goals and Objectives

This section helps track progress and provides guidance for improving the employee’s performance.

  • Past Goals Achievement: Reviews if the employee met previously set goals.
  • New Goals: Outlines new goals for the next evaluation period.

 

Section: Overall Evaluation

The purpose of this section is to summarize the employee’s strengths and identify the areas that need improvement.

  • Overall Rating (The rating can range from 1, equal to Below Expected Level, to 5, equal to Excellent Performance):
  • Strengths: Outlines the key areas that the employee excels.
  • Areas for Improvement: Identifies those aspects that require further development.

 

Section: Comments and Future Goals

In this section, you could add further comments and identify the future goals that the employee can set. The goals can be either short-term or long-term goals, and you can also mention suggestions for developing specific skills or the need for additional support or training.

Section: Employee Feedback

This section allows employees to share their thoughts on their performance, career development, and the evaluation process, ensuring a two-way communication approach.
